Basic Annual Services

Wellness Visit I

• Thorough physical medical examination and consultation with your veterinarian
• Vaccines based on risk factor analysis
• Fecal test to check for the presence of intestinal parasites
• Body condition score evaluation

Wellness Visit II

• Thorough physical medical examination and consultation with your veterinarian
• Comprehensive lab work including a complete blood count, extended chemistry profile, urinalysis, and thyroid hormone level. Cats will also be tested for FeLV and FIV
• A customized patient and life-stage assessment profile based on age, breed, lifestyle, lab results, and preventative health care goals will be developed with the Pet Wellness Report

Premium Plan

Adult Pet Plan

• Basic package allows for 2 exams/consultations annually); Premium allows for unlimited exams/consultations during the course of the plan period.
• Vaccinations based on AVMA recommendations including: Canine DHPP, Canine Lepto4, Canine Bordetella, Feline FVRCP, and Rabies
• Fecal parasite and heartworm testing
• Annual wellness blood work including: CBC, chemistry panel, T4 thyroid testing, and urinalysis
• Annual Feline Leukemia (FELV) and Feline Immunodeficiency Virus (FIV) screening for cats
• Pet Wellness Report – a comprehensive Health Risk Assessment individualized for your pet's breed, age, lifestyle, and more
• Up to 12 months of heartworm and intestinal parasite prevention with
  • 6 months of additional flea and tick protection (dogs)
  • 2 spa days
  • 2 days of boarding

Puppy Pet Plan

Everything you need to have a healthy, happy puppy who will be part of your family for years to come at an affordable monthly price.

• Up to 4 wellness visits including all vaccines, intestinal parasite tests, deworming, and nail trims as outlined in our Puppy Wellness Package.
• Up to 15 months of heartworm, intestinal parasite, and flea prevention
• 6 additional months of flea and tick protection
• Spay or neuter procedure using the safest anesthesia. A professional spay/neuter procedure includes: a comprehensive physical exam, pre-anesthetic health screening, IV catheter and fluids, sterile spay/neuter procedure, pain management package (includes medications given before, during, and after the procedure) and a night of hospitalization
• Microchip placement
• 2 spa days and 2 days of boarding

Kitten Pet Plan

Getting kittens off on the right path to live a happy and healthy life as a member of the family.

• Up to 4 wellness visits including all vaccines, intestinal parasite testing, deworming, and nail trims as outlined in our Kitten Wellness Package
• Feline leukemia virus (FELV) and feline immunodeficiency virus (FIV) testing
• Spay or neuter procedure using the safest anesthesia. A professional spay/neuter procedure includes a comprehensive physical exam, preanesthetic health screening, IV catheter and fluids (for spay), sterile spay/neuter procedure, pain management package (includes medications given before, during, and after the procedure)
• Up to 15 months of heartworm, intestinal parasite, and flea prevention with Revolution
• Microchip placement
• 2 spa days and 2 days of boarding

Dental Plan Add-on

This can be added to the Adult Pet Plan packages.

• Includes safety, comfort, and IV catheter and fluids
• Closely monitored anesthesia
• Basic level 1 or 2 dental cleaning
• Grade 3 or 4 cleanings, extractions and medications will require additional fees

Senior Pet Plan Add-on

This can be added to the Adult Pet Plan packages.

Includes 3 of the following tests upon your veterinarian's recommendation in the plan year:

• X-Rays (3 film) OR abdominal ultrasound
• Echocardiogram imaging (ECG)
• Tonometry
• Blood pressure check
• Cardiac Brain Natriuretic Peptide (BNP) test
• CBC, Chemistry panel, T4, and Urinalysis
